Non-Extraction Solutions We Use First
Before recommending an extraction, we explore every available option to create the space your teeth need. Here are the most common approaches:
- Interproximal reduction (IPR): A small amount of enamel is gently removed from between certain teeth to create room for movement without losing a full tooth.
- Arch expansion: Aligners can be designed to gradually widen the arch, creating more space for crowded teeth to shift into proper alignment.
- Distalization: Molars are strategically moved backward to open up room in the front of the arch, which can resolve crowding without extraction.
- Incisor proclination: In some cases, slightly tipping the front teeth forward creates enough space to align the arch comfortably.

When an Extraction May Be the Right Call
Some cases genuinely require an extraction before aligner treatment can begin. According to research published in the National Institutes of Health’s PubMed Central, tooth extraction is typically indicated when the discrepancy between available and required space in the arch exceeds 8 mm, such as in cases involving severe crowding or significant protrusion. In these situations, removing a tooth, most often a premolar, creates the space the aligners need to move your teeth safely and predictably.
Severe Crowding and Bite Discrepancies
Patients with moderate to severe crowding may have teeth that are so tightly packed there simply is not enough room in the arch to align them without first making space. Trying to force movement without adequate room can lead to unpredictable results or even damage to the roots and surrounding bone. An extraction resolves this by opening a defined amount of space that the aligners can then methodically close.
Protrusion Cases
When the upper or lower front teeth sit too far forward relative to the jaw, aligners alone may not be able to move them back sufficiently. Extracting a premolar allows the front teeth to retract into a balanced position, improving both the bite and the overall facial profile. This is a well-established approach and does not compromise your smile’s long-term health.

Most patients start wearing their aligners within a few weeks, once the extraction site has healed adequately.
